Super Postal will supply all the necessary forms to fill out. You will need to provide two (2) forms of I.D. for each adult (over 18 years old) listed on the mailbox (see PS-1583 for acceptable forms of ID). A one-time application fee of $20.00 is required to open a private mailbox.

Package Storage Fee = $5/day/package
Mail Forwarding = $5/occurrence + shipping

Heavy Volume #1 = $2.00/pick up
Heavy Volume #2 = $5.00/pick up
Heavy Volume #3 = $10.00/pick up
Unusually heavy volume = based on audit
Add’l Names 5 – 7 = $15/month
Add’l Names 8 – 10 = $25/month
